We offer three easy options for registering, including online with a special association partner discount code. The Care Continuum Alliance will verify your membership status and process your registration within five business days.

3. PURCHASERS SAVE WITH SPECIAL RATES

The Care Continuum Alliance is proud to offer reduced-rate registrations for the following categories.

  • Full-time providers or employees of health plans, hospitals, health systems.
  • Full-time employees of federal, state or local government (contractors do not qualify).
  • Full-time employees or students at a college or university and not employed by any health care organization.
  • International attendees who permanently work and live outside of the United States.
  • Executive responsible for purchasing and administering health benefits within an organization that is not in the business of providing health care products or services. Example of titles in this category include: vice president of human resources, director of benefits, benefits manager and chief medical officer.

To receive these discounts, you must submit your registration to the Care Continuum Alliance by mail or fax (202-478-5113). Online registrations do not qualify for this discount. The Care Continuum Alliance will verify your eligibility and process your registration within five business days; additional documentation to verify eligibility may be required.

4. GROUP PRICING FOR CARE CONTINUUM ALLIANCE MEMBERS

For every two paid attendees from your organization who register for The Forum 11, a third employee receives a free Forum registration. Groups must submit their registration forms by mail or fax and pay together to receive the savings—online registrations are ineligible for group discounts. Fax team registrations to (202) 478-5113. Registrations for workshops, special rates and day passes are excluded from this promotion. Group discount restrictions:

  • Please note that group discounts are available only to Care Continuum Alliance members in good financial standing.
  • Each group registrant must complete a registration form.
  • The three forms must be received together with complete payment information to be eligible for the promotion.
  • Group discounts are available only for The Forum 11 registrations. Registrations for symposiums, special rates and day passes are excluded from this promotion.
  • Group discounts are not available for online registrations.
  • If, due to cancellations, a group falls to fewer than three registrations, the group discount will be forfeited and the standard registration fees will be assessed. Registrations not meeting these requirements will be responsible to pay any balances due at onsite registration.

5. BRING A GUEST

We are pleased to offer a guest pass designed to provide attendees' spouses, significant others and adult children (at least 21 years of age) access to the Exhibit Hall for evening receptions for $25 per reception. A guest pass may be purchased only at onsite Registration by the attendee who is registered for The Forum 11. Employees of health care related companies are ineligible for guest passes. A guest pass does not include conference materials, luncheons or provide access to educational sessions.
