Realizing the Potential:
Improving Population Health In Accountable Care

Primary care practices and hospitals will form the nexus for accountable care models and will need resources and proven methods for assessing patient populations, stratifying health risk, structuring successful solutions and measuring and reporting outcomes. This special education and networking event, co-located with the Forum Sept. 6 and 7, will explore how population health management supports systems of accountable care and the tools and experience care coordination and health management offer to help physicians, hospitals and other providers succeed in the new era of accountability.

In two half-day sessions, prominent experts in collaborative care and population health will present critical information on federal policy and the outlook for physician practices, health risk assessment and stratification, data analytics and integration and other key considerations for accountable care models. Registration for this event may be purchased separately from Forum 11 registration.
