Outstanding Leadership in Collaborative Care Award
Care Continuum Alliance Awards

Gain recognition for your work to include diverse stakeholders in wellness and chronic condition care: Submit a nomination for the Care Continuum Alliance Outstanding Leadership in Collaborative Care award.

This award, to be presented Sept. 7 to 9 at The Forum 11, the Care Continuum Alliance annual meeting, in San Francisco, will recognize an individual, team or organization for innovative models of care delivery that:

  • Model has a chronic management focus;
  • Model is sustainable and replicable;
  • Model serves a broad population;
  • Model has representation from multiple stakeholders and community representation; and
  • Model has payment innovation and a strong business model.

CORE CRITERIA

  1. Commitment: Exhibits measurable organizational commitment to population-based care.
  2. Program Design: Successfully employs science and evidence-based practice guidelines in the design of population health improvement programs.
  3. Implementation: Establishes excellence in the extended execution of population health improvement programs.
  4. Innovation: Demonstrates improvements to the science of population-based care by applying innovative strategies that overcome challenges and barriers.
  5. Outcomes-based Orientation: Provides robust, objective evidence of financial and health outcomes.
  6. Scope and Potential Impact: The degree to which the success of the initiative and effort could potentially impact the population health improvement community and change the lives of those served by it.
  7. Vision and Leadership: Demonstrates commitment, vision and leadership to the principles of population health improvement.
