EXHIBIT BOOTH RATES
Sign-up early for extras—Reserve and pay for your booth on or before May 6, 2011, and you will save $500 and be able to include, at no additional cost, a small promotional item with your company logo in the conference tote bag. Promotional items must be supplied at your expense.

COMPLIMENTARY BENEFITS
Each 8' x 10' booth space includes:
- Standard furnishings, including a booth sign, 8' high back drape and 3' high side drape, two (2) plastic side chairs, a 6' draped table, daily aisle cleaning, and wastebasket.
- Twenty-four hour perimeter security.
- Listing in the program guide.
- An enhanced listing on the conference Web site including logo, 50-word description and contact information.
- Pre-conference, single-use registrant mailing list in July 2011.
- Post-conference single-use registrant mailing list in September 2011.
- Complimentary Forum registrations for up to three (3) individuals—workshops require additional registration fees.
HOST PRODUCT DEMOS ON THE SHOW FLOOR
New! Highlight your new products and services in a specially designed theater on the show floor during Exhibit Hall hours. Demonstrate your product, show a video or offer a Q&A session—you decide. Each product demo is 20 minutes long, including a 5-minute question & answer session. The title of the product demonstration and presenter(s) must be submitted to the Care Continuum Alliance by Friday, Jun. 3, 2011. ($2,000)
EXHIBIT HALL CHALLENGE
New! Every registered attendee will receive a "Challenge" to visit participating exhibitors, get their entry form stamped and be eligible for the grand prize drawing—a $1,000 American Express gift card. Each participating company will receive a unique stamp for its spots on the grid and a fact to share with attendees about prevention, wellness or chronic care (e.g., "1 in 3 U.S. adults could have diabetes by 2050"). At the end of the Networking Reception Thursday, Sept. 8, attendees will be selected to answer questions corresponding to the exhibitor-supplied facts. The first attendee to correctly answer a question will win the Challenge prize. ($500)
