Volunteer Opportunities 2011

The committees, subcommittees and workgroups identified below represent the groups that currently have openings for 2011. New groups may be added as projects develop throughout the year. Space is limited for each workgroup listed below and available only to Care Continuum Alliance members. Once the slots are filled, a waiting list will be developed. Each workgroup listed below meets on a regular basis via conference call. Active and consistent participation is expected. Contact jmay@carecontinuum.org with questions or to volunteer.

Industry Research Landscape and Blueprint—Develop a comprehensive landscape of the current published and non-published research efforts in the Industry along with the perceived gaps in the current efforts and suggestions on how to develop efforts to begin to address these gaps.

Population Health Management—Two workgroups will be created to address the following objectives:

  1. Expand on the work already completed in the area of population health management measures to include a set of cross cutting measures with specifications.
  2. Review the current body of PHM/wellness/health promotion research to determine additional areas of work that would include advancing knowledge and understanding of productivity and the use and impact of incentives. Review current literature and begin to develop recommended strategies for the deployment and evaluation of population health management programs in ACOs, PCMHs, and other new models of care delivery.

Engagement—There will be several workgroups focused on the area of engagement. Possible workgroup topic areas are below. If you have an interest in working on the topic of engagement or any of the areas specified below, volunteer for the general engagement work and the group will be divided at a later date.

  1. Explore the area of engagement including the identification of published literature on the topic. Identify best practice strategies for various populations.
  2. Develop a set of case studies, and ultimately endorse measures on the topic. Additional areas could include expanding the set of definitions developed for Volume 5 and continuing the operational measures work.
  3. Expand engagement into the areas of behavior change and best practices/strategies currently deployed in the industry.

Care Transitions—The Transitions of Care Workgroup will continue to work on the development of a comprehensive best practice framework for the transitions associated with a heart failure population that will include evidence and case studies.

International Work—The International Committee will explore the possibility of developing relationships with international care management companies and conducting an exploratory survey of international efforts among the current Alliance membership.

Market Trend—Now in its third year, the workgroup will oversee the development of the 2011 Care Continuum Alliance Market Trend report. This will include survey review and refinement, data analysis, and report development.

Behavioral Health—The workgroup will develop a tool kit focused on reviewing the topic of behavioral health and identifying ways to integrate.
