Care Continuum Alliance Learning Events Explore Intersection of Population Health Management and New Models of Accountable CareWASHINGTON, D.C.—Caring for patient populations is new territory for many providers gearing up to take the lead in emerging accountable care models. In a new online and onsite learning series, the Care Continuum Alliance will demonstrate how population health management strategies and tools can support success in accountable care. Two webinars and a live conference Sept. 6 and 7 will showcase proven wellness and health management tools and resources—long recognized for their value in the employer health benefits arena—for physicians, physician groups, hospitals and others pursuing success in this new care model. The Care Continuum Alliance, a 200-member industry association for wellness, prevention and health management, today announced that David K. Nace, MD, Vice President, Medical Director, McKesson Corp., will chair the webinar portion of the learning series, "Realizing the Potential: Improving Population Health in Accountable Care." The series will kick off with two webinars and culminate with two half-day learning sessions immediately preceding the Care Continuum Alliance's annual meeting, The Forum 11, Sept. 7-9 in San Francisco. "Primary care practices and hospitals will form the nexus for accountable care models, and will need resources and proven methods for assessing patient populations, stratifying population health risk, structuring successful solutions and measuring and reporting outcomes," said Care Continuum Alliance President and CEO Tracey Moorhead. "The population health management industry is poised to support the work of providers, and can offer the ready resources for data analytics and data integration that will be demanded for success in these new models." Earlier this year, the Care Continuum Alliance published a toolkit on population health management resources for accountable care organizations—its initial offering in the broader initiative now continuing with the learning series. The toolkit is available as a free download from the Care Continuum Alliance website, www.carecontinuum.org. The learning series features two initial webinars:
