Membership Categories

Care Continuum Alliance members receive generous benefits, including access to the members-only services and other online tools; the weekly eNews newsletter; print and online subscriptions to our peer-reviewed journal, Population Health Management; communications updating members on Care Continuum Alliance legislative and regulatory advocacy efforts; announcements about educational programming; and discount offers for population health improvement products and partner conferences.

Members have the opportunity to serve on Care Continuum Alliance standing and ad hoc committees and can hold elective office; and receive voting privileges at the annual business meeting.

Listed below are the categories of membership and the descriptions and fees associated with each category. Membership fees are based on the joining organization's total revenue for the most recent fiscal year. Membership dues are not refundable, transferable or pro-rated.

Population Health Management Providers—Organizations that provide wellness, disease or care management or population health services or education. Dues are assessed at the greater of annual dues level of the parent company or the population health management provider company.

Population Health Management Support Organizations—Includes organizations providing support services to population health management providers including software solution providers, health information technology innovators, remote patient monitoring device manufacturers, electronic medical record systems, personal health record suppliers, predictive modeling and other stratification services. These organizations do not offer care management.

Individual membership is available on a limited basis and only to those that do not qualify for membership under their organization; i.e. independent allied health professionals; full-time academicians; full time independent specialty consultants; a full-time employee or student at a college or university and not employed by any health care organization. (Documentation to verify eligibility is required to process individual memberships. Examples of documentation required for Allied health professionals and Independent specialty consultants include: DBA filing, business license, letters from clients, or letterhead and a business card. Requirements for other categories might include student enrollment or government employee verification.)
