Population Health Improvement

Welcome to the Quality Improvement Case Studies Registry. The Care Continuum Alliance (CCA) developed this registry to gather and disseminate information on quality in population health improvement programs, such as wellness and disease management. Through this registry, CCA seeks to promote widespread access to best practices information for members, the larger population health improvement community and all stakeholders in chronic disease prevention and care.

The Quality Improvement Case Studies Registry is part of a broader quality initiative spearheaded by the CCA Quality Improvement Steering Committee that also includes an annual quality awards program and collaboration with other health care quality organizations. The registry is the first element of a developing area of this site devoted to promoting the contributions of population-based interventions to health care quality.

What will CCA do with this information?

The registry will be available to all stakeholders and will give industry representatives an opportunity to submit and highlight quality improvement efforts in a condensed format. Organizations delivering programs at any level of intervention—wellness, chronic care management, palliative care and others—are encouraged to submit case studies. CCA will post here, for public viewing, select submissions to the registry.

Please note: personal data requested during the submission process—submitter's name and e-mail address, for example—are for submission verification and CCA internal use only, and will remain confidential. CCA will not display this information or share it with outside parties. To protect the integrity of the database, only CCA staff will be able to edit and post submitted case studies.

How do I enter case studies?

Submissions may be made using an online form. Members of the CCA Quality Improvement Steering Committee will review all submissions for completeness and clarity. Submissions should be as complete as possible and include a program overview and, if available, outcomes. CCA might ask the submitting organization to provide revisions or clarifications as needed before posting the case study.

How do I view case studies?

You can display an online listing of all case studies in the Registry or search the Registry with predefined keywords and user-supplied phrases.
